 commence la réunion en nous présentant la structure du ministère de la santé et 
l’historique de la création du numérus clausus.

L’ONDPS : travaille surtout avec la DREESS et avec la DGOS sur les projections et le Numérus 
Clausus(NC) 
Le RPPS : répertoire des professionnels de santé, répertorie  4 professions médicales (Médecins, 
dentistes, pharmaciens, sages-femmes). Il est traité à la fois par CNOM et DRESS 
 
Historique du NC 
nous explique que le NC a été créé en 1972 suite à la situation chaotique 
engendrée par 1968 qui avait vu le nombre de diplômés en médecine démultiplié et qui dès lors 
était supérieur à la capacité de formation dans les hôpitaux. Ainsi à l’origine, le NC n’était pas un 
dispositif budgétaire de régulation. 
 C’était engendré par une peur des médecins de perdre leur activité. Suite à cette inflation de 
médecins après 1968, le NC a encore été diminué entre la fin des années 80 et 1990 (médecins, 
dentistes,). La situation a cependant été différente pour la maïeutique et les pharmaciens.  
Les effets de cette baisse du NC se font sentir particulièrement aujourd’hui 
Dans les années 2000, il y avait par rapport au NC des opinions différentes assez radicales. D’une 
ceux qui voulaient augmenter le NC et ceux qui voulaient le baisser en disant que « l’offre crée 
demande », plus il y aurait de médecins, plus il y aurait de malades. 
Cependant le développement de nombreuses innovations, le changement d’attitude générationnel 
vis-à-vis du travail du médecin (plus de temps partiel) et le vieillissement de la population, lequel a 
entraîné une augmentation de demandes de soins, ont fait que le besoin en personnel médical a 
évolué.  

 

On se retrouve donc avec une relative pénurie pour les 10 années à venir. 
Implications d’une augmentation voire suppression du NC 
Qu’il s’agisse des médecins ou des dentistes, une augmentation du nombre d’étudiants impliquera 
une augmentation du nombre de place de stages pour la formation « pratique ». or ce nombre 
commence à être très réduit. Pour ce qui concerne les dentistes, le NC actuel correspondrait  au max 
des capacités de formation au niveau des stages 
Des solutions sont testées pour palier à cette pénurie de places de stages : 
Développement des maîtres de stages en villes (en ambulatoire) mais alors il faut prévoir des 
mesures d’aide aux médecins. La loi santé prévoit une aide pour tout médecin prenant un stagiaire. 
En odontologie problème est différent car ils ont juste besoin de bouches donc des mannequins, des 
morts, ou des personnes qui vont à l’hôpital peuvent permettre au dentiste de pratiquer et 
d’apprendre. 
Les stages dans les cliniques privées sont compliqués à obtenir car un stagiaire coûte cher au 
professionnel en pratique privée car il est moins rentable. Ainsi ces dernières offrent peu de 
débouchés 
 
Suppression du NC : De quoi parle t’on ? 
De fait la suppression du NC ne résoudrait pas tout, et face à une embolisation du système (pénurie 
actuelle d’ophtalmologues, système d’urgences surchargés) d’autres leviers que l’augmentation du 
nombre de médecins peuvent être mis en place. C’est notamment ce qu’essaye de faire le plan 
Santé 2022 en développant par exemple certaines infirmières vers un profil d’infirmière en pratique 
avancée (spécialisées) qui reprendraient certaines tâches de médecins ou en élargissant certaines 
tâches simples (par exemple vaccination) aux infirmières ou à d’autres professions (pharmacien). Les 
médecins ont aussi poussé à la création d ‘une nouvelle profession d’assistante médicale qui 
reprendrait certaines tâches simples des infirmières.  
La France ne veut pas d’une logique purement quantitative d’augmentation du nombre de médecins 
Besoin médical si pas d’offre, ne s’exprime pas si pas dans sécu pas remboursé et donc pas 
demandé. 
Derrière l’idée de la suppression du NC, il y a l’idée de changer le mode de sélection des étudiants. 
 
Prochaine étapes 
Suite à l’annonce de la suppression du NC, un groupe de travail a été créé sur les parcours 
d’étudiants dans les trois premières années.

Sur la suppression du Numérus Clausus et la réforme des études médicales 
Les masseurs-kinésithérapeutes, les médecins, les dentistes ne sont pas hostiles à la suppression du 
Numerus Clausus (NC) , qu’ils estiment de toutes façons désormais superflu car détourné du fait de 
l’existence de la procédure de reconnaissance européenne des qualifications. 
Dans le cadre du contournement du NC les dentistes constatent la création d’écoles de dentistes 
privées en Roumanie ou Espagne indépendamment des besoins du pays, dans lesquelles des français 
vont obtenir des diplômes, puis revenir en France. C’est ainsi qu’à NC constant la population de 
dentistes augmente de 18% chaque année. 
On peut noter aussi que certains étudiants ayant échoué à la PACES font eux le choix d’aller étudier 
en Afrique  plutôt que dans un autre pays de l’Union européenne et reviennent demander la 
reconnaissance de leurs qualifications  ensuite en France. Cependant les médecins estiment à 
environ 4000 le nombre de médecins diplômés d’Afrique n’ayant pas obtenu la reconnaissance et 
exerçant sous la tutelle d’un médecin français inscrit à l’ordre. L’absence d’enregistrement à l’ordre 
de cette population fait qu’ils sont difficiles à suivre. 
Sur la réforme des études médicales 
Les médecins sont conscients qu’une réforme des études médicales est nécessaire pour éviter le 
couperet de la PACES humainement difficile et pour éviter la sélection de médecins uniquement sur 
base de certains profils (plutôt matheux). Pour eux il serait plus approprié d’établir des quotas par 
région/département (NC par territoires) et d’aller vers une sélection progressive tout au long des 
années avec l’établissement de passerelles. Par ailleurs l’examen national classant va être supprimé 
et remplacé par un examen au fil de l'eau.  

 

L’offre de stage en CHU (hôpitaux universitaires) tend à se réduire. Cela peut poser des problèmes. 
En général, les médecins considèrent  les capacités des universités limitées. 
Les masseurs-kinésithérapeutes demandent à faire partie des commissions de réflexion sur la 
réforme des études médicales. Les médecins y sont déjà.  
Les masseurs-kinésithérapeutes  veulent rester dans la voie universitaire. Ils veulent que le BAC+5 
soit acté. 
Les infirmiers – kinés – infirmiers - pédicures – podologues souhaitent tous passer par l'université 
dans le futur. 
Déserts médicaux 
La suppression du NC n'est pas un remède pour les déserts médicaux et un éventail de mesures 
diverses devrait être prises pour supprimer les déserts médicaux. L'ordre des médecins est attaché à 
une régulation par territoire. 
Europe 
Les dentistes demandent la mise à jour de l'Annexe V et un contrôle de la qualité dans la continuité 
du diplôme mentionné dans l’annexe V Æ faille pour la bonne marche de la directive. Par 
comparaison, dans le cadre du CETA, un contrôle régulier via une accréditation limitée dans le temps 
a lieu. En général ils demandent une approche européenne de coordination plus soutenue. 
Les médecins soulignent que de grandes différences existent entre les formations de médecins 
spécialistes en Europe (années d’études varient de 3 à 8 ans). Par ailleurs les actes réalisés varient à 
l’intérieur des spécialités : en Allemagne par exemple les urologues n'opèrent pas. 
Les dentistes demandent à la Commission européenne si elle a des chiffres sur l'accès partiel en 
Europe.  Les masseurs- kinésithérapeutes notent que l’accès partiel n’est pas numérisé en Europe 
(pas partie de la Carte professionnelle européenne) 
Question 2 : Besoins en terme de formation 
Infirmiers: On passe à l'université cette année et on a besoin d'investissements pour offrir une 
formation de qualité. 
Les masseurs-kinésithérapeutes indiquent qu’il manque 3000 équivalents temps plein à l'hôpital car 
en salarié hospitalier, un masseur-kinésithérapeute gagne beaucoup moins. Les masseurs-
kinésithérapeutes compétents partent en libéral et donc les offres de stage de qualité en hôpital se 
réduisent. Ceci est dû au fait que les IFMK sont privés bien souvent. Les masseurs-kinésithérapeutes 
doivent donc rembourser les études et ne peuvent se permettre de rester simple salariés. 
Dentiste : Avec la suppression du NC il est inévitable que les besoins en formation vont augmenter. 
Il y a par ailleurs un besoin en formation paradentaires. 
 
 

 

Offre de soins - Organisation des professions de santé 
Pour les médecins: le développement de Communautés Professionnelles Territoriales de Santé (CPTS) 
devrait permettre d’augmenter l’offre locale. Cependant sur les 1000 CPTS qui étaient prévues en 
2016, à ce jour seulement 200 ont vu le jour. Par ailleurs pour les masseurs-kinésithérapeutes qui ont 
besoin de surface souvent la participation aux charges de l’infrastructure sont plus importantes. 
Concernant certain soins on est dans des situations dramatiques (psychiatrie). 
On peut aussi noter que les masseurs-kinésithérapeutes demandent l’accès direct au patient et non 
pas seulement sur ordonnance du médecin.  
Pour pallier à certaines carences en matière d’offres de soins on envisage de faire bouger les activités 
réservées des professions en permettant à certaines infirmières dites en pratique avancée de faire 
des actes habituellement réservés au médecin, en créant une profession d’assistant dentaire de 
niveau 2 , ou en créant une profession d’assistante médicale ayant vocation à assister le médecin à 
un niveau différent que les infirmières. 
Les dentistes annoncent effectivement qu’une réflexion a lieu sur la création d’une assistante 
dentaire de niveau 2 : la création d’un nouveau métier (qui serait similaire à la profession 
d’hygiéniste dentaire). La définition des actes reste cependant à faire.  
Les infirmiers sont irrités voir inquiets de la création de la profession d’assistante médicale, qui selon 
eux empiète sur la profession d’infirmière. Pour les médecins, le but n’est pas de remplacer les 
infirmiers dans la mesure où elle ne sera pas formée à faire des soins mais à préparer les visites du 
médecin. 
Question 6 : modalités d’exercice en société 
Pour les médecins c’est en général le régime de la société d’exercice libéral (SEL) qui est choisi. Pour 
l'exercice interprofessionnel ce sont les sociétés interprofessionnelles de soins ambulatoires (SISA). 
Ce dernier est un modèle intéressant mais qui a ses limites.  Elle doit compter au moins deux 
médecins et un auxiliaire médical parmi ses associés. Selon les médecins les regroupements 
interprofessionnels actuels ne sont pas adaptés. 
55% des dentistes exercent en Société 
Ouverture du capital : Pour les ordres dès lors qu’il y a ouverture du capital, on arrive dans le modèle 
des chaînes et  il y a très vite dérive vers la recherche du rendement. Les ordres sont en faveur du 
capital réservé aux professionnels de santé. 
Le système des maisons de santé est une initiative intéressante mais le modèle devra sans doute 
encore être retravaillé. 
Système d’alerte européen – accès à IMI – Carte professionnelle européenne 
IMI est surtout utilisé pour demander le champ des compétences 
Système d'alerte ne fonctionne toujours pas selon les professions de santé présentes.   

 

Par ailleurs la procédure carte professionnelle européenne fonctionne pour les masseurs – 
kinésithérapeute. Cependant la procédure n’est pas forcément plus simple que la procédure papier. 
Par ailleurs pour certains pays, les diplômes avant d'être envoyés ne sont pas toujours contrôlés, les 
cartes d'identité non plus. Notamment pour les documents venant de pays sans entité 
d'enregistrement certifiée cela peut être un problème. Les pharmaciens estiment que le système est 
encore dans une phase de rôdage. Pour eux la décision de reconnaissance du diplôme précède la 
procédure d’inscription à l’Ordre avec laquelle elle ne se confond pas.

Main issues and conclusions: 
The Strategy 'Ma Santé 2022', that was presented by the President and the Minister of Health in late September 
2019. 'Ma Santé 2022' is to foster the coordination and cooperation between various health professions in 
primary, ambulatory specialist and inpatient hospital care, and for better access to quality healthcare throughout 
the country.  
While the government considers the strategy as a major step towards a better coordination of care, better access 
to care in the regions and an opening up and reform of medical and paramedical professions, the health 
professions in particular were somewhat critical, in particular on the creation of new professions in health care 
(advances nurses and medical assistants, for instance) and the doctors were critical towards a transition of certain 
competences to nurses, while the nurses are in favour. However, the overall need for reform was not put into 
question. 
It was also interesting to see that the federation of public hospitals as well as the federation of private hospitals 
where somewhat critical of the reform and see themselves possibly footing the bill of the shift towards primary 
and ambulatory care, that successive French governments have been pursuing for years. Still, the FHP's position 
seemed a bit more flexible, given that they are offering a selection of the more cost-effective interventions (e.g. 
day surgeries) in an ambulatory setting. 
I got the impression that the success of 'Ma Santé 2022' will to a large degree depend on the active support and 
involvement of the medical professions and a clear legal and organisational framework that will need to be 
adopted in the coming months and years to set the right incentives for efficiency, care coordination  and an 
equitable  provision of health care (in particular general healthcare) in all parts of the country. Electronic health 
services play a key role in the government's strategy, but the stakeholders we saw seemed to put not so much 
attention to them, yet.  Again, the stakeholders will need to follow and possibly adapt. 
The reform of the French healthcare system is just moving on to another stage…

FRENCH MINISTRY OF SOLIDARITIES AND HEALTH

Regional disparities in the availability of healthcare staff, described as "medical deserts" are an important 
political issue.
  France ranks slightly below average in Europe in terms of medical density, with a ratio of 3.1 
doctors per 1,000 inhabitants1,  and access to care has become difficult in some geographic areas, mainly in rural 
zones, peri-urban zones and 'sensible'  urban zones, given the lack of sufficient practising doctors.  
The French Ministry of Health underlined that there was no immediate and miraculous solution to the 
problem, given it has to do with demography (27% of doctors are 60 or over, and there is a relative lack of 
trained younger doctors, esp. in general practice), changes in the gender composition of doctors (about 60% are 
now women) and in the work habits of doctors (a general increased desire for a better reconciliation of work and 
family life, i.e. avoiding long hours and overtime). 
The implementation of the 2017 plan for access to care in the regions is showing first result. The Ministry 
highlighted 4 of them: 
- So far up to 600 physicians benefited from financial support to the establishment of medical practice in an 
underserved areas: Doctors can receive up to €50 000 if they set a practice to work for at least 4 days a week. 
- Up-grading the nursing professions. As from 2018 the profession of 'nurse in advanced practice' will be created. 
Education of the first nurses in a 5 years course (3 years plus 2) has just started in 2018. The Ministry expects 
between 500 and 600 students to be educated within the next 3 years. These nurses, who are expected to work 
mainly in an independent own practice, will be allowed and able to perform advances nursing task , including 
task so far limited to the practice of a medical doctor. 
- Coordinated practice in pluri-professional teams (exercice coordonné  dans équipes pluri-professionels). 
Medical professionals in a geographical area can cooperate to improve patient pathways. They can cooperate in 
so-called medical homes ('maisons de santé) where health professionals that cooperate stay independent or they 
work in a medical centre (centre de santé) where they receive a salary. The government has so far spent  €350 
million for the creation of medial homes. There are up to 539 medial homes now in France. 
- Traineeships in general medicine in underserved regions for medical students: Students receive an extra salary 
of  € 200,  plus financial support for travel and logging during their traineeship in an underserved region. 
 
The Strategy 'Ma Santé 2022', that was presented by the President and the Minister of Health in late September 
2019' is to foster the  coordination and cooperation between various health professions in primary, ambulatory 
specialist and inpatient hospital care, and for better access to quality healthcare throughout the country.  
One short-term measure consists in hiring 400 family doctors in so-called "medical deserts". 
Also the abolition of the numerus clausus (which is, in fact, an annual quota of students allowed to study) is 
expected to contribute to mitigating the challenge of medical deserts, but only in the very long run, as medical 
studies in France take many years, according to the Ministry of Health. 
'Ma Sante 2022' foresees also the creation of up to 1000  professional territorial health communities 
(Communautés professionelles territoriales  de santé – CPTS). These communities, which were created in the

2016 healthcare modernisation law, are regional associations of different medical,  paramedical and social care 
professions  to ensure, e.g. short waiting times, access to a general practitioner, the cooperation with local 
hospitals or care at the patients' home.   
Hospitals in France will be classified into in three categories: local health care, specialized care and ultra-
specialized care—each focusing on its priorities, to optimize patient care. Between 500 and 600 local hospitals 
('hôpitaux de proximité') should become operational by 2022, offering basic services in hospital care. They 
should (if they are public hospitals) be integrated with the territorial hospital groups (groupements hôspitaliers de 
territoire, GHT
) that were created by the 2016 law to improve the cooperation between public hospitals within a 
defined geographic area.  Areas of cooperation are, for instance, to converge to unique information systems in 
GHT, the coordination of training inside a GHT and joint procurement by all providers in a GHT.  
 Up to date there are 135 GHT. They are very diverse in membership and geographical coverage, ranging from 2 
participating hospitals to more than 20. The process of establishing GHT is currently stagnating, and the 
government is considering to boost the process, e.g. by new provisions according to which  members in a GHT 
(where each hospital stays so far an independent entity in particular in financial terms) should in the future 
foresee solidary financing among each other. Another reflection is to give to university clinics not just a 
coordination function but a lead function in the GHTs.  According to the Ministry, the medical staff  in the 
hospitals are rather open to the reform and welcome the possibilities of increased cooperation across 
establishments and professions. The hospital managers and administrators, however, are critical. 
To note that GHT are for public hospitals only, but the government encourages also private hospitals to develop a 
coordinated cooperation with GHTs. 
 
E-Health 
The 2016 to 2020 eHealth strategy aims to develop digital platforms, strengthen security, foster innovation and 
develop 'big data' to support, for instance, the development of new tools for monitoring or diagnosis. E-
prescribing  has not yet been implemented in France, yet 
In its recent health system transformation strategy, the French government announced its intention to provide to 
all users of the healthcare system with a personalised digital space by 2022, including, for instance, electronic 
health records or for making doctor's appointments. 
Teleconsultation, i.e. remote consultations between patients and their healthcare practitioners have been available 
in France as part of the coordinated care pathway since the 2018 Social Security Financing Bill. As of September 
this year reimbursement by the national health insurance of a telemedical consultation is possible   subject to 
requirements of being conducted by video-transmission, being integrated within the coordinated care pathway 
and the patient already being known to the teleconsulting physician. To encourage further uptake practices are no 
longer required entering into contracts with regional health agencies or set-up an agreement among the doctors 
conducting the telemedicine. It is also foreseen to extend telemedical consultations to advanced medical practice 
as of February next year. 
Pharmaceuticals 
In the previous years, some efficiency gains in pharmaceutical spending have been achieved, but high-cost 
originator medicines and the low use of generics remain an important challenge. On the high cost of originator 
medicines, a working group on reform of medicines evaluation is to present results and proposals in spring 2019. 
Between 2006 and 2016, the generic market share in volume nearly doubled in France, rising from 14.5 % to 
26.5 %. However, this share remains below the EU average and far below the share in countries such as the 
United Kingdom, Germany and Spain. 
The government insists that the generic substitution rate is already more the 80% in 2017. The draft law on the 

financing of social security 2018 foresees that as of 1 January 2020, an originator medicine that is substitutable 
by a generic is reimbursed only at the price of the generic (even against the will of the patient) unless there are 
objective medical reasons for taking the originator medicine that have to be confirmed by the doctor. This should 
save up to € 100 million in the short term. The draft law also seeks to clarify the conditions under which a doctor 
can declare an originator medicine as 'non-substitutable by a generic', which have the be defined  by the national 
agency for the security of medicines and medical products  (agence nationale de sécurité du médicament et des 
produits de santé"
).

The French hospital Federation gathers more the 1 000 public health care establishments (hospitals) and more 
than 1 000 public retirement homes. The Federation represents almost 100% of public hospitals.  
 
Although FHF is in general positive about 'Ma Santé 2022', they are critical towards the ensuing implementation 
measures, such as the draft law on financing social security.  They think that the hospital sector has already been 
footing the bill of French healthcare reform for too long, experiencing successive cuts and financial deficit. They 
regard the primary care sector  to be in the more favourable position,  receiving more  political  and financial 
support. 
 
While, according to FHF, the budget for the national health insurance is to increase by 2.5% from 2018 to 2019, 
the hospitals will be required to save € 800 million. While the hospital sector has again spent less than its 
financial objective in 2018 (€105 million less than foreseen), the savings in hospital care are used to promote 
primary and ambulatory care. 
 
FHF would like to see the DRG rates for hospitals to be increased by 1 % and supports an overall reform of 
payment systems. 
 
FHF demands that lump-sum payments, similar to the DRG system that is established in French hospital care, 
should also be applied in primary and ambulatory care, in order to contain what they consider a cost inflation in 
primary care. They also think that primary care sector should be obliged to provide on-call services as the 
hospitals do, and also the primary and ambulatory care in general would benefit from a reorganisation in France.  
 
In general, FHF supports the shift to more ambulatory care  in France, underlining the increasing activity of 
public hospitals  in day-surgery, ambulatory consultations and hospitalisation at home. Still, they think that 
hospitals are footing the bill for the 'virage ambulatoire' of successive governments.

FHP represents 1 030 private medical establishments in France which cover for 32 % of activity in the French 
hospital sector and 28% of the costs incurred for hospital care in France, which FHP sees as a sign of the private 
hospital's efficiency (However, they usually do not provide for certain high cost services, such as emergency 
rooms, but this is, according to FHP, mainly due to the government being reluctant to authorise emergency rooms 
in private hospitals). 
 
The hospital sector in France has been in constant transition over the last years, with more international providers 
entering the private market and the overall reduction of beds in hospitals. 
 
Similar to FHF, FHP considers the hospital  sector to foot the bill for savings in French healthcare, with DRG 
rates being reduced effectively over the previous 6 years. Therefore, FHP also supports the demand of all hospital 
federations in France for a 1% increase for 2019. 
 
FHP  welcome 'Ma Santé 2020' in so far as it opens the door for better cooperation between professions and 
establishments in healthcare, creating more effective patient pathways, increasing quality of care in particular for 
patients with chronic conditions and strengthening the geographical access to care. Still, they think the strategy 
does not provide enough incentives for innovation and efficiency, also in the ambulatory and primary care sector.
