I thank you very much for the links provided. They are very useful and helpful for me.
I do not know if only a question without to be attached to a document is eligible to be treated by EDPS. However I will try to address my question:

Do you consider that this form and lack of special Privacy Statement http://fra.europa.eu/en/about-fra/regist... is in line with the applicable requirements. I requested via this website some documents from FRA and during the exercise I was requested to fill these forms. But I cannot see the Privacy statement for access to documents. Neither the name of processor of my data.

An undated "Information Note for Data Subjects Access to Documents" - uploaded in the meantime in FRA website http://fra.europa.eu/en/data-protection-.... At the time of writing my email of 04/10/2013 was not there.

FRA reaction by publishing Information Note is laudable.
