Generic questionnaire for prior notifications of article 25 of Regulation No 45/2001

Dear European Commission Data Protection Officer,

Under the right of access to documents in the EU treaties, as developed in Regulation 1049/2001, I am requesting documents which contain the following information:

The Data Protection Officer - DPO has drawn up a structured questionnaire for the purposes of the prior notifications of article 25 of Regulation No 45/2001 (hereafter 'the Questionnaire'). It appears that:

- the questionnaire includes some 35 questions

- data controllers, and possibly also Data Protection Coordinators, have been fillng in such questionnaires in the context of consulting with the DPO and formally notifying to the DPO the article 25 of Regulation No 45/2001 prior notifications.

The term 'Associated Documents' hereunder refers to any document directly associated with the Questionnaire such as 'instructions' how to fill in the questionnaire, notes to data controllers about the Questionnaire etc.

Copies of the following documents drawn up by the DPO are kindly requested:

1: The year 2006 version of the Questionnaire and the Associated Documents

2: The year 2007 version of the Questionnaire and the Associated Documents

3: The year 2008 version of the Questionnaire and the Associated Documents

4: The year 2009 version of the Questionnaire and the Associated Documents

5: The year 2010 version of the Questionnaire and the Associated Documents

6: The year 2011 version of the Questionnaire and the Associated Documents

7: The year 2012 version of the Questionnaire and the Associated Documents

8: The year 2013 version of the Questionnaire and the Associated Documents

In my view, none of the exceptions of article 4 of Regulation No 1049/2001 may be invoked to refuse total access.
