Paperwork when officials leave the Commission

Under the right of access to documents in the EU treaties, as developed in Regulation 1049/2001, I am requesting a copy of all standard forms which are commonly sent for completion to staff members, including permanent officials and temporary contract agents, when they leave the Commission's employment (whether through personal choice, redundancy, end of contract etc). I would further like to receive any other standard information which the Commission commonly sends to departing staff members which reminds them of their ongoing obligations under Article 16 of the Staff Regulations.
